The Yahtzee Score Sheet, Explained
A classic five-dice score sheet has 13 boxes: six upper-section boxes (Aces through Sixes) that score matching dice, and seven lower-section boxes: 3 of a Kind, 4 of a Kind, Full House (25), Small Straight (30), Large Straight (40), 5 of a Kind (50), and Chance.
The upper section
The upper section is the counting half of the sheet. Each box scores only the dice matching its number:

If your six upper boxes total 63 or more, you earn a 35-point bonus. Sixty-three is exactly three of each number, which is why experienced players guard their Fives and Sixes.
The lower section

Below the boxes, a full sheet also tracks the 5-of-a-kind bonus (100 points for each additional five of a kind after your first 50) and the grand total.
The score sheet in Feltzee

Quick answers
What is the bonus box on the score sheet?
There are two: the upper bonus (35 points for reaching 63+ in the upper section) and the 5-of-a-kind bonus (100 points per additional five of a kind after the first, scored at 50).
Can each box be used more than once?
No. Every box is filled exactly once, which is why a game lasts exactly 13 turns.
